Please have your veterinarian fill out the form below and email us the completed document to ovcslah@uoguelph.ca. Please note that the referral form has to be reviewed by our rehab team and then one of our client service associates will reach out to you with a day and time for your initial consultation with Dr. Durzi and her rehab team.
