Please have your veterinarian complete the form below and email the completed document to ovcslah@uoguelph.ca. Once we receive the referral form, it will be reviewed by our rehabilitation team to assess your pet’s needs. After the review, one of our client service associates will reach out to you to schedule your initial consultation with Dr. Durzi and her rehab team. During this consultation, we’ll discuss your pet’s condition, treatment options, and create a personalized rehab plan tailored to their recovery.
